Student Request to Schedule Exams at DRC

Students requesting to schedule exams at the DRC must fill out a separate form for each course.

  • DRC testing hours begin at 9:00 a.m. and end at 4:00 p.m.
  • ALL exams and quizzes must be scheduled at least five (5) business days in advance before 4pm.
  • Professors must approve the date and time of your exam before you schedule your exam at the DRC.
  • The DRC can only proctor 40 exams per day, so schedule as soon as you can.

Classroom Relocation Form

The DRC will work with the UIC Office of the Registrar to relocate classrooms when necessary. Ideally, requests for classroom relocation should be submitted two weeks before the start of the semester to avoid disruptions. Students who wish to request a classroom relocation can submit their requests via the form.


Interpreter/CART Request and Cancellation Forms

Requests for interpreter or Communication Access Realtime Translation (CART) services can be made via the Request Form.

Cancellations for single interpreter or Communication Access Realtime Translation (CART) services can be made via the Single Cancellation Request Form.


Note Taker Agreement Form

Interested in being a Note Taker for a student registered with the Disability Resource Center?  Please fill out the Note Taker Agreement Form.

The Disability Resource Center (DRC) coordinates with UIC students to share class notes with classmates who are registered with the DRC and may require assistance with note taking. Note takers play an important role in providing DRC students with clear and comprehensive notes. Note takers are not expected to provide academic assistance, such as tutoring or aiding students with course assignments.
