Documents and Forms
Students requesting to schedule exams at the DRC must fill out a separate form for each course.
- DRC testing hours begin at 9:00 a.m. and end at 4:00 p.m.
- ALL exams and quizzes must be scheduled at least five (5) business days in advance before 4pm.
- Professors must approve the date and time of your exam before you schedule your exam at the DRC.
- The DRC can only proctor 40 exams per day, so schedule as soon as you can.
The DRC will work with the UIC Office of the Registrar to relocate classrooms when necessary. Ideally, requests for classroom relocation should be submitted two weeks before the start of the semester to avoid disruptions. Students who wish to request a classroom relocation can submit their requests via the form.
Interpreter/CART Request and Cancellation Forms
Requests for interpreter or Communication Access Realtime Translation (CART) services can be made via the Request Form.
Cancellations for single interpreter or Communication Access Realtime Translation (CART) services can be made via the Single Cancellation Request Form.
