Alternate Format Request Form

Alternate format requests are not retroactive. Course material will be converted from the week the request is submitted. 

Turn-around time for this service depends on the nature of the request and the time during the semester it is received. If you have any specific concerns about turn around time of your request, please email the alternate format team at accesstext@uic.edu.

Once requested materials are ready for use, they will be uploaded for you to the UIC cloud platform, Box folder created by the Alternate Format Team. If you need
